The Heart Of The Oak (HOTO) runeword is extremely popular among casters due to the fact that it provides a plethora of useful bonuses to all classes and builds, particularly when fighting demons. Aside from maces, any mace or staff class weapon can be used to create this weapon, with the exception of maces. Because flails can be wielded with one hand, they are extremely popular among players who want to use their other hand for a shield. This makes flails particularly useful for this purpose. If you're playing as a Sorceress and you come across a 4-socket staff with the perfect set of modifiers, you'll want to make a Heart Of The Oak staff instead of using the staff you've already obtained.
It is necessary to have exactly four sockets on the base weapon when playing Heart of the Oak in order for the runeword D2R KoVexPulThul to be spelled out correctly in the game. Always remember that you must add the runes to the weapon in the exact same order that they were received, or you will not be able to obtain Heart Of The Oak. As a result, begin with the Ko rune, followed by the Vex rune, the Pul rune, and finally the Thul rune to complete the sequence.
Flails with four sockets can be found anywhere in Act IV Normal and later, or you can find a normal flail and add sockets using the Horadric Cube (only flails found in Act IV Normal and later will have a chance to get four sockets), or you can use Larzuk after completing the Siege of Harrogath quest line in Act V.
Because it drops in Act IV on Normal difficulty and continues to drop throughout the game, Thul is the easiest of the HOTO runes to find. Thul is the easiest of the HOTO runes to find. Ko is the next most straightforward, but it is significantly more difficult to locate, than the previous two characters. It begins to drop during Act II on the Nightmare difficulty; however, you'll have a better chance of finding it at the Hellforge on either the Nightmare or Hell difficulties, or receiving it from The Countess on either difficulty. When playing Nightmare, Pul behaves in the same way, though it doesn't start dropping until Act IV of the game. In addition, Vex is extremely difficult to come by in large quantities. There is no way to obtain it from either the Countess or Hellforge, so you'll have to wait for a random drop somewhere on Hell difficulty to come along.
